Baptism Form
Infant Baptisms (NB if you are an Adult see * below,) also sometimes called christenings are usually held at the 10.30am Parish Mass on Sunday once a month. As well as our existing welcoming congregation, we also might have other families occasionally to have their children baptised as well.
Please fill in the details below and click to return the form to the Church Office and we will get in touch to confirm.
We do ask parents attend our Sunday Worship at least twice before we confirm a date.
You are also expected to meet with the priest in advance of the date, to talk about the baptism and the promised that parents and God parents make. Click here for those promises.
There is a clear expectation that both Parents and Godparents are themselves baptised.
That both parents clearly understand that Baptism in the Church is a serious undertaking and not just a ‘nice’ thing to do nor even a family tradition.
If you feel you can’t make the vows and declarations at a Baptism, please ask Fr Simon about a Thanksgiving for the Birth of a Child Service.
* Adult Baptism can also take place - but it assumed that you have been coming to church for a few weeks and have made a decision to be baptised after talking with the Priest. He will give you an Adult Baptism Application Form. So don’t complete this form.
If you are unsure or need clarification about any of this please talk with/email Fr Simon. He is friendly and wants to work with you.
